Good news: The DSWD has digitized most of its application process. However, the (also known as the DSWD Clearance Request Form ) is available both online and at physical DSWD Field Offices.
This shift to full online processing, implemented starting February 2025, means that applications are now processed exclusively through the online platform at mta.dswd.gov.ph. While DSWD field offices may offer assistance during the transition period, particularly for those unfamiliar with online platforms, the application itself must be completed online.
